Rides at an Indiana county fair have been removed after a girl was thrown from one attraction, according to officials and local news outlets.

A little girl was thrown from the “Rock and Roll” ride, which goes up and down, and sustained injuries, according to reports from WHET, WFIE and the Bradenton Herald

In the wake of the incident, a fair-goer named Anna Deorto shared in a Facebook post that the “girl right next to me flew out” of her seat during the ride.

“She went headfirst. Like, her body just, like, went, falling through with her head,” Deorto told WFIE, adding that the ride didn’t immediately stop after the girl flew out.

“I’m holding on, like trying to get people’s attention, waving my arms,” Deorto recalled. “‘Hey, she fell out. Stop the ride. You need to stop this ride. The seatbelt’s not latched you guys. She fell out. She needs help.’”

She also claimed to the outlet that the rider operator never checked their seatbelts.

PEOPLE reached out to the Dubois County Sheriff's Office but did not immediately receive a reply.

David Hosick, with the Indiana Department of Homeland Security (IDHS), told PEOPLE in a statement that the head of their inspections unit reached out to the owner of the rides, who "indicated the specific operator of the ride had failed to properly secure a group of girls into one of the carriages" of the ride. He said that the girl "received minor injuries" after she "fell out."

"She was given a bandage and ice pack and left the scene with her family and friends. No ambulance or additional treatment was requested or required at the scene," he said, adding that the ride was shut down after the incident.

Hosick noted that the "Rock and Roll" ride was last inspected on June 11, and during the inspection two of the rider carriages were "deemed unusable" due to safety violations.

These included maintenance violations, like "missing belt fasteners" and "damage to rider carriages" and "assembly and wielding" violations like "warped" diamond plate flooring and "broken rivets."

He noted that the ride "cannot be utilized in any fashion until it passes an IDHS compliance inspection."
