In addition to all the other goodies introduced in Diablo IV: Vessel Of Hatred, there is also a new random dungeon, the Undercity. In this guide, we’ll walk you through everything you need to know to gain access and start your own runs.

The Undercity: What You Need To Know

There are two levels of access that you can obtain for the Kurast Undercity dungeon in Vessel of Hatred.

The first level requires you to be Level 15 and complete the Undercity: Wail of the Forgotten quest, which only unlocks the ability to run the Undercity dungeon with one boss, and no modifiers (which means no added rewards, too). You will also need to have completed enough Vessel of Hatred story missions to have met Ormus The Undying. Once you have reached the required point, the purple The Kurast Undercity quest will be added to your Feature Unlock Quest log.

The second level of access is granted once you also complete Undercity: The Promise and Undercity: Starving Pride, which unlocks the full roster of random bosses for the Undercity, as well as the ability to use Tributes to enhance dungeon runs. These Tributes are consumables which add challenging modifiers to your runs and extra rewards for completing them.

Once you have access to the quest, head to Kurast, then enter the portal on the eastern side of the city.

Go back to town and talk to Councilor Sihek in the southern corner, where he is speaking to a family that has left their house, which they attest is haunted.

Go inside and inspect the shining objects, then head outside and return to Sihek (by the Undercity entrance) and inform him of your findings.

He’ll task you with seeking out a memorial to the southwest of the city, so go inspect it and Alia will speak to you.

Return to the Undercity entrance and speak to Sihek, then use the brazier to create a portal to the Undercity: Ziggurat.

At this point, you can run the Undercity: Ziggurat dungeon as many times as you like, but you’ll be limited to the Ziggurat area, which only ends in a fight against Alia, so let’s continue to unlock the other possible areas and bosses.

How Does The Kurast Undercity Dungeon Work?

The way the Undercity works is:

  • You’ll proceed through three levels, each with a portal located somewhere within which leads to the next level

  • You’ll start with 100 seconds and 0 Attunement, and you’ll gain Time (orange) and Attunement (blue) by defeating enemies as you proceed

  • If the timer hits 0, the run will end and you’ll need to leave and start another run

  • If you reach the portal in Level 3 before the timer runs out, you’ll be taken to the boss level and the timer will stop

  • Defeat the boss to claim your rewards and complete the run, with higher levels of Attunement leading to greater rewards

After completing all Undercity missions (listed below), you’ll unlock the ability to enhance your runs for extra challenge and reward

If you are having trouble reaching the end before the timer runs out, simply run past all the enemies and find the portal. Jumping through to the next level gives you extra time, and you are not required to defeat ANY enemies to reach the boss.

Defeat Alia, Kurast’s End

Head inside, find each portal as quickly as you can, then fight Alia in the boss stage. She’ll cause a ring of either green or red energy to encircle you during the fight, then spawn waves of those same colors.

Run through the hazard that matches the circle around you to avoid taking damage. Keep it up and kill her to complete the quest, then head outside and speak to Sihek by the Undercity entrance again.

After speaking to Sihek and inspecting the corpses near him, go outside the city to the southeast and find the Golden Snakes. Kill them and loot their Golden Blood until you have 10, then a place called Ennead’s Folly will appear. Go inside and explore the Mage Clan ruins.

Once you reach the final chamber, inspect the chest and the book, then pick up the dagger and return to Sihek in Kurast and speak to him.

After he reveals the existence of a possible progenitor, he’ll send you back to the Undercity, so return to the brazier and start another run. This time, it will send you to the Temple district, and the final boss will be Yoche, The Golden.

During the fight, Yoche will channel power through the serpent statues, so knock them down one by one to cut him off and continue the fight. If your damage is high enough, you can kill him before he does this, however. Good luck!

Once you’ve defeated Yoche, you’ll complete Undercity: The Promise, and begin Undercity: Starving Pride, the final quest in the chain to unlock the full potential of The Undercity.

After defeating Yoche, head outside and find Sihek in the southwestern part of Kurast. Speak to him and he’ll direct you to a home in the southeastern corner of the city. Speak to him there, then head inside the home to find and kill the Infected Lacuni Scout.

Speak to Sihek again and he’ll task you with obtaining 10 Insect Husks from Afflicted Lacuni.

Head directly southeast of Kurast to find them, then kill them until you’ve gathered a sufficient number. After that, seek out a small encampment to the west and inspect the shining objects, then return to Sihek.

He’s near the entrance to the Undercity, and speaking to him will unlock the Cave District. Inspect the brazier and start another run to reach the Cave District.

Reach the end and slay the Lacuni boss, Longtooth, to complete the quest.

Head back to the entrance to the Undercity dungeon and Sihek will have left. Inspect the tablet he left, then head to the council chambers in the northeastern section of Kurast to meet him and speak with Ormus.

After that, speak with Sihek again to unlock the full potential of the Undercity, as well as acquire three ‘Tributes,’ which can be used to add modifiers (and increase rewards) from Undercity runs. They can be found in the Dungeon Key tab of your character sheet.
